SMS Consent for Patient Appointment Communications

Patients may receive transactional text messages from InnovateHealth on behalf of their clinic. These messages support appointment operations and clinic workflow follow-up.

1. Program scope

InnovateHealth supports clinic-approved operational messaging. Messages may be sent by a partner clinic via InnovateHealth after a patient provides their mobile number and consents through an appointment, intake, registration, website, or staff-assisted workflow.

  • Appointment confirmations
  • Appointment reminders
  • Pre-visit intake form links
  • Reschedule or cancel updates
  • Callback coordination
  • Related clinic operational support for an existing appointment or requested clinic communication

2. Consent language

When patients opt in to patient appointment communications, the consent language is:

I agree to receive transactional text messages from InnovateHealth on behalf of my clinic about appointment confirmations, reminders, intake forms, reschedule/cancel updates, callback coordination, and related clinic operational support. Message frequency varies. Message and data rates may apply. Reply STOP to opt out and HELP for help. Consent is not a condition of treatment or purchase.

SMS opt-in data, mobile numbers, and consent status are not sold or shared with third parties or affiliates for marketing or promotional purposes.

3. Sample messages

Hi [First Name], this is [Clinic Name] via InnovateHealth. Reminder: your appointment is on [Date] at [Time]. To reschedule, use [Link] or call [Phone Number]. Reply STOP to opt out.
Hi [First Name], this is [Clinic Name] via InnovateHealth. Please complete your pre-visit intake form before your appointment: [Link]. Reply HELP for help or STOP to opt out.

4. What is not included

This patient appointment messaging program is not used for promotional marketing. Standard SMS is not used for diagnosis, treatment advice, test results, prescription decisions, emergency instructions, or sensitive clinical details.

If you have a medical emergency, call 911 or contact local emergency services. Do not use SMS, website forms, or InnovateHealth workflows for emergency medical situations.

5. Help, opt-out, and terms

  • Reply STOP to opt out of text messages.
  • Reply HELP for help.
  • Message frequency varies by clinic workflow and appointment activity.
  • Message and data rates may apply.
  • Consent is not a condition of treatment or purchase.
